1. Tsing Yi MTR Station (Assembly Time: 9:30am / Dismissal Time: 4:15pm)
2. Noah’s Ark Hong Kong Main Gate (Assembly Time: 9:45am/ Dismissal Time: 4pm)
(Once the assembly and dismissal location is chosen on this enrollment form, no changes will be allowed.)

# Upon the completion of this application form, applicants who are not yet members of the Noah’s Academy program will be enrolled to the program automatically. • Enrollment is on first-come-first-served basis. Enrollment will only be confirmed once the payment has been made.
• Payment by VISA / MASTERCARD only.
• A particular session will be cancelled if there are less than 15 applicants by the application deadline. An email of session cancellation will be sent to the applicants' parents. Paid course fee will be refunded through credit card.
• Payment will not be refunded should the participant quit or not be able to join the workshop under any circumstances or reasons. The fee is non-refundable and non-transferable.
• The applicant will receive a participants’ notice and special arrangement application form via email before the session commences. If participant has any food allergy, special health condition or special needs, please state in the form for arrangement. Ma Wan Park Limited reserves the rights to reject applications that are beyond the service scope.
